The Tear Drainage System
The tear production and drainage system plays a crucial role in maintaining eye health. Tears are produced by the lacrimal gland located under the upper eyelid. These tears lubricate the eye and are drained through small openings called puncta, located on the upper and lower eyelids.
From the puncta, tears travel through the canaliculi, leading into the lacrimal sac, and finally into the nasolacrimal duct, which channels the tears into the nose. This process explains why crying often results in a runny nose.
What Causes Blocked Tear Ducts?
In most infants, a membrane blocking the nasolacrimal duct dissolves shortly after birth. However, failure of this membrane to open can result in a blocked tear duct, causing tears to overflow and potentially leading to infection.
Symptoms include constant tearing, pus-like discharge, and crusted eyelids, particularly noticeable upon waking.
Treatment Options
In about 85% of cases, blocked tear ducts resolve without intervention. Initially, the focus is on preventing infection and promoting natural resolution. Antibiotic eye drops may be prescribed to manage infections, though they do not address the blockage itself.
Home Care
Parents are often advised to “milk” the tear duct by applying gentle pressure to the tear sac, which can sometimes open the blockage.
Surgical Intervention
If the blockage persists, a procedure called probing may be recommended. This involves using a thin wire to clear the obstruction, with a success rate of about 95%.
For more complex cases, additional procedures such as balloon catheter dilation or silicone tube insertion might be necessary.
