If you need a Georgia DMV Vision Form completed, please fill out the top portion with your information and email the form to clinical@eyephysicians-surgeons.com. There is a $25 form completion fee. A staff member will contact you to collect payment before the form is processed. Please allow up to 48 hours for completion. You may also choose to complete the vision screening at the DMV at no charge.
